GENERAL appointment information
All appointment information will be securely emailed to you at the time of booking your appointment. Please open the secure link that is attached to your email and complete the attached New Patient Form prior to 8AM the day before your appointment.
PLEASE NOTE THE FOLLOWING:
• Effective April 4, 2023: Masks are welcomed, but no longer mandatory. Masks still provide an added layer of protection. If you are experiencing any mild cold or flu-like symptoms, please advise our office staff & please wear a mask during your appointment to protect others. If you are experiencing significant symptoms, we kindly ask you to reschedule your appointment. Thank you for your cooperation.
• Do NOT arrive earlier than 10 minutes prior to your appointment time
OUR SHOE POLICY:
Our office has a NO OUTDOOR SHOES policy. We ask that you please remove your footwear at the main entrance when you arrive. You may wear your socks or bring along a pair of clean slippers to wear inside the clinic.
WHAT TO BRING/WEAR:
• Knee(s) - a pair of shorts or pants that can be rolled up over your knee(s)
• Shoulder(s) - a tank top or t-shirt appropriate for examination
• Hip(s) - a pair of shorts, comfy pants, or leggings (nothing restricting)
X-RAYS:
A specialized series of X-rays may be required prior to your appointment. Please remember to have these done at least one hour before your appointment. Failure to have these x-rays done will result in cancellation of your appointment.
Your requisition has been faxed to Prairie Skies Medical Imaging (see locations below). You do not need an appointment to have these x-rays done.
